Sports Physical / Wellness 

Student will get sports physical form given to them at the event to take to their school

School Required Vaccines can be administered 

Consents will need to be signed

Insurance will be billed, but parent/guardian of student/athlete will not receive a bill

Please bring insurance cards

If you don’t have insurance, we will have Community Health Workers on hand to assist with signing up for OHP

If unable to attend the event, please call 541-677-7200 to get your child scheduled for their wellchild check including sports physical
