Lori Hart

Lori Hart joined the Evergreen Family Medicine team in August 2023, bringing over 25 years of experience as a nurse practitioner. With dual certifications as a Psychiatric Nurse Practitioner from the University of Kansas and a Women’s Health Nurse Practitioner from the University of Utah, Lori has a diverse and comprehensive approach to patient care. Additionally, she holds a PhD in Education from Capella University, which has further enriched her ability to guide her patients and work as a faculty member in helping to develop the next generation of nurse practitioners.
Lori’s passion for mental health care shines through in her holistic, integrative approach, where she combines both medication management and therapy to help individuals achieve a balanced, fulfilling life. She provides mental health services to individuals 13 years and older and has a special interest in supporting women through significant life transitions, such as postpartum and menopause.
Originally from Southern California, Lori has followed her husband’s career as an optometrist across the country, living in Tennessee, North Carolina, Utah, and Alaska before finding their way to Roseburg. After enduring a brutal winter with temperatures plunging to -20 degrees and snowstorms dropping up to 31 inches, the temperate climate of Roseburg was an irresistible draw for them. While they’re excited to call this community home, the only thing missing is their family—and they’re working on that!
Lori and Rick have been married for 43 years, raising four children along the way, and now enjoying the joy of five grandchildren. They share their home with their beloved 100 lb dog, Dr. Watson.
Lori is excited to bring her expertise, humor, and commitment to her patients at Evergreen Family Medicine, helping them navigate both mental and physical health with care and compassion.
